How do I Enroll For Refractive Surgery?  

Each soldier applying needs to meet the minimum eligibility requirements and be aware of the priority candidate list:  

Minimum Eligibility Requirements:

          You must have at least 18 months of active duty remaining at the time of your surgery.

          You must have at least 12 months duty remaining in the same occupation and unit at the time of surgery

          You can not have any adverse personnel actions pending.

          You must have your Commander's Authorization for surgery.

          Your refractive error must meet the criteria for surgery.

          You must agree to comply with the requirements for post-operative follow-up exams.

          You must be able to remain CONUS for at least 60-90 days post-surgery.  

You must be 21 years or older.

Priority Candidates:

          Priority 1: Deploying Soldiers

          Priority 2: Combat arms soldiers not currently deploying

          Priority 3: All others are space available

If you meet the above requirements, proceed with the following:

Step 1: Download/Print the PRK/LASIK Application, Patient History Questionnaire, Managed Care Agreement, and Commander's Authorization Form.

Step 2: Fill out all your information according to the directions on the cover sheet and take the Commander’s Authorization to your Unit Command for signature.

Step 3: If you wear contacts, discontinue use until otherwise told to do so.  You must be out of contacts for at least 30 days before your pre-op appointment and continue to be out of them for surgery.

Step 4: Take the signed Commander’s Authorization, PRK/LASIK Application, to your local DoD eye care provider to schedule a pre-op appointment. 

Step 5: The Joint War Fighter Refractive Surgery Center at Wilford Hall offers refractive surgery services to eligible members of the military. You can send your packet via mail, email or fax:

Step 6: Please keep a copy of all information for your personnel records.  You will be notified of your status by e-mail within two weeks after submission.
